by land and sea..

as of this posting, there is no direct route from Manila to Caticlan Antique via the sea route but there is some bus line in Cubao that services the route by land although it takes a long time, maybe a day to get there. Once in Caticlan Jetty port, ride a boat that will take you to the island. Or take this route instead, take a bus from Cubao, Quezon City in Metro Manila, to go to Batangas pier then take a ferry to go to Roxas City in Mindoro then another ferry to Caticlan. You might as well check the schedule of 2 major shipping lines, they sometimes have a route from Manila to Caticlan, just google NegrosNavigation and WGA superferry for the schedule as they include this route during the summer from april to late june.
